Privacy Policy

1. Who We Are

Integrity Senior Care is a private nursing practice operated by Payal, RN, BScN, Registered Nurse, licensed with the College of Nurses of Ontario, located in London, Ontario, Canada (“we,” “us,” or “our”).

2. Scope and Applicable Law

This Privacy Policy governs the collection, use, and disclosure of personal information and personal health information (“PHI”) obtained through our website (integrityseniorcare.ca) and consultation intake process. It is written in compliance with:

  • PHIPA: Personal Health Information Protection Act, S.O. 2004, c. 3 (Ontario)
  • PIPEDA: Personal Information Protection and Electronic Documents Act, S.C. 2000, c. 5 (federal)
  • CASL: Canada’s Anti-Spam Legislation, S.C. 2010, c. 23

3. What Information We Collect

When you submit a consultation request through our contact form, we collect:

  • Your name
  • Email address
  • Phone number
  • A description of the health or care situation (this may constitute personal health information under PHIPA)
  • Your preferred contact time (optional)

We also collect standard server-side technical data (IP address, request timestamps) for rate limiting and security purposes. This data is not retained beyond operational necessity.

4. Why We Collect It

We use your information solely to:

  • Respond to your consultation enquiry
  • Assess whether our services may meet your needs
  • Contact you to arrange a consultation call

We do not use your information for marketing purposes, and we do not send unsolicited commercial communications. Any follow-up email will be a direct response to your inquiry.

5. Where Your Information Is Stored

Our website is hosted on Vercel Inc. (San Francisco, CA, USA), a cloud hosting provider. Form submissions are transmitted to us via Resend (a transactional email service). Standard server access logs are retained by Vercel per their data retention policies.

By submitting our contact form, you acknowledge that your information will be transmitted to and processed in the United States by these third-party infrastructure providers. Both Vercel and Resend operate under industry-standard data security practices.

Important: Email is not a secure channel for transmitting sensitive health information. We encourage you to share only the minimum information needed to describe your situation. For sensitive clinical matters, call us directly at (226) 884-6767.

6. Who We Share It With

We do not sell, rent, or share your personal information with any third parties for marketing or commercial purposes.

We may disclose your information only if required by law, a court order, or to protect the safety of an individual in accordance with PHIPA’s permitted disclosures.

7. How Long We Retain It

Consultation inquiry information is retained for up to 2 years from the date of submission to support continuity of care discussions and respond to follow-up inquiries. After this period, records are securely deleted.

If you become a client, clinical records are retained in accordance with PHIPA’s minimum retention requirements for health records (10 years from last entry, or until the age of 28 if the client was a minor at the time of care).

8. Your Rights

Under PIPEDA and PHIPA, you have the right to:

  • Access the personal information we hold about you
  • Request correction of inaccurate information
  • Request deletion of your information (subject to legal retention obligations)
  • Withdraw consent to our use of your information at any time
  • File a complaint with the Office of the Information and Privacy Commissioner of Ontario (IPC)

9. Cookies and Analytics

Our website uses Vercel Analytics and Vercel Speed Insights to collect aggregate, anonymized performance data (page views, load times). These tools do not use third-party tracking cookies and do not collect personally identifiable information.

By using our website, you consent to this anonymized analytics collection. No cookie consent banner is required for analytics that do not identify individual users or set persistent tracking cookies.

10. Security

All data transmitted between your browser and our website is encrypted via TLS (HTTPS). We implement security headers including Content Security Policy, HTTP Strict Transport Security, and anti-clickjacking protections. Contact form submissions include bot protection and rate limiting.

No internet transmission is 100% secure. We take reasonable technical and organizational measures to protect your information but cannot guarantee absolute security.

11. PHIPA Compliance Statement

Integrity Senior Care recognizes its obligations as a health information custodian under the Personal Health Information Protection Act (PHIPA). We collect personal health information only for the purpose of providing or facilitating health care services. We do not collect more PHI than is necessary for this purpose.

We designate Payal, RN, BScN as the Privacy Officer responsible for ensuring PHIPA compliance. Any privacy breach will be assessed and reported in accordance with PHIPA’s mandatory notification requirements.

12. Changes to This Policy

We may update this Privacy Policy from time to time. Material changes will be posted on this page with an updated “Last updated” date. Continued use of our website after any changes constitutes acceptance of the revised policy.
