03Will you replace our family doctor or specialist?
No. We add a continuity RN to your existing team, monitoring between appointments and escalating to your physician with concrete clinical data when something shifts.
04How often will the nurse visit for chronic condition oversight?
Visit frequency is set by the care plan and the condition, from weekly oversight to several times per week during an unstable period, and adjusts as the situation changes.
