1. Agreement to Terms

By accessing or using the website located at integrityseniorcare.ca(“Site”) or by engaging with the services offered by Integrity Senior Care(“we,” “us,” or “our”), you agree to be bound by these Terms of Service. If you do not agree, please do not use this Site or our services.

2. Services Description

Integrity Senior Care provides private, RN-led nursing oversight and care coordination services for seniors and their families in London, Ontario and surrounding areas. Services are delivered by Payal, RN, BScN, a Registered Nurse licensed with the College of Nurses of Ontario.

Services may include but are not limited to: post-discharge nursing oversight, chronic disease management support, palliative care support, medication reconciliation, care coordination, and family guidance. All services are provided within the scope of practice of a Registered Nurse as defined by the College of Nurses of Ontario and the Regulated Health Professions Act, 1991.

3. Not Medical Advice

The information provided on this Site is for general informational purposes only and does not constitute medical advice, diagnosis, or treatment. Nothing on this Site should be construed as establishing a nurse-client or healthcare provider-patient relationship.

Always seek the advice of your physician, nurse practitioner, or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on this Site.

4. Private Pay Services

All services provided by Integrity Senior Care are privately paid and are not covered by OHIP (Ontario Health Insurance Plan) or any provincial health insurance program. It is your responsibility to verify whether your extended health benefit plan provides any reimbursement for private nursing services.

Detailed receipts suitable for benefit plan submission are provided upon request. Specific service fees, payment schedules, and cancellation policies are outlined in the individual service agreement signed at the time of intake.

5. Consultation Requests

Submitting a consultation request through our contact form does not constitute a binding service agreement or guarantee of services. A formal service agreement is established only after a direct consultation and mutual written consent.

By submitting a consultation request, you consent to being contacted by Integrity Senior Care regarding your inquiry in accordance with our Privacy Policy.

6. Limitation of Liability

To the maximum extent permitted by applicable Ontario and Canadian law, Integrity Senior Care and Payal, RN, BScN shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of this Site or reliance on any information contained herein.

Our total liability for any claim arising from use of this Site shall not exceed the amount paid by you for services in the twelve months preceding the claim, or $100 CAD if no services have been purchased.

7. Intellectual Property

All content on this Site, including text, design, graphics, and the Integrity Senior Care name and logo, is the property of Integrity Senior Care or its licensors and is protected by Canadian copyright law. You may not reproduce, distribute, or create derivative works without our express written consent.

8. Prohibited Use

You agree not to use this Site to:

  • Submit false, misleading, or fraudulent information
  • Attempt to gain unauthorized access to any part of the Site or its infrastructure
  • Harass, threaten, or harm any individual
  • Violate any applicable federal, provincial, or local law
  • Engage in automated scraping or data harvesting

We reserve the right to refuse service to any person for any reason consistent with applicable law.

9. Third-Party Links

This Site may contain links to third-party websites. These links are provided for your convenience only. We have no control over the content of those sites and accept no responsibility for them or for any loss or damage that may arise from your use of them.

10. Governing Law and Dispute Resolution

These Terms are governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein, without regard to conflict of law principles.

Any dispute arising from these Terms or your use of our Site shall first be addressed through good-faith negotiation. If unresolved, disputes shall be submitted to the courts of competent jurisdiction in the Province of Ontario.

11. Changes to These Terms

We may update these Terms at any time. Material changes will be posted on this page with an updated “Last updated” date. Continued use of the Site after any changes constitutes your acceptance of the revised Terms.
